Hydroxyapatite and fluoride: not either/or, but both
The internet urges you to pick a side: "natural hydroxyapatite" versus "chemical fluoride". In reality these are two different mechanisms that reinforce each other — and a good paste contains both.
What is hydroxyapatite
Tooth enamel is 96% made of a mineral with the formula Ca₁₀(PO₄)₆(OH)₂. That mineral is hydroxyapatite. The one added to toothpaste is chemically identical to it — only milled down to micro- and nanoparticles.
Put simply: the tube contains the very substance your tooth is made of. Hence the name — a biomimetic, one that "imitates nature".
Two mechanisms, two tasks
Hydroxyapatite physics
- Builds into microcracks and enamel defects, smoothing the surface
- Fills the dentinal tubules — those microscopic channels through which cold "reaches" the nerve
- Creates a calcium-phosphate reserve on the tooth
- Does not cause fluorosis — it is safe even to swallow
Fluoride chemistry
- Builds into the enamel's crystal lattice, turning it into fluorapatite
- Fluorapatite is more resistant to acid — it dissolves at a lower pH
- Forms a fluoride depot that keeps working after brushing
- Suppresses plaque bacteria
The key point. Hydroxyapatite replenishes lost mineral. Fluoride makes enamel more resistant to the next acid attack. The first is about repair, the second is about armour. Removing either one means giving up half the protection.
Why hydroxyapatite helps with sensitivity
Sensitivity appears when enamel has worn away or the gum has receded and the dentine is exposed. Dentine is riddled with thousands of microtubules that lead straight to the nerve ending. A sip of cold water shifts the fluid in these tubules — and you get a short, sharp pain.
Hydroxyapatite particles physically plug the openings of the tubules. The mechanism is purely mechanical, with no strong alkalis and no "freezing" of the nerve — so the effect builds up gradually, over 2–4 weeks of regular brushing.
What about fluoride-free HAP pastes?
Such pastes exist, and the hydroxyapatite in them does work. But an anti-caries effect comparable to fluoride has not been demonstrated for them: systematic reviews confirm caries reduction specifically for fluoride pastes (from 1000 ppm — by roughly 23%, according to Cochrane).
In Belarus, where the water is low in fluoride, giving up fluoride means losing the only proven source of protection. It is wiser not to choose between the ingredients but to take a paste that contains both.
